*（*）投標(biāo)人須按招標(biāo)文件第一冊(cè)第四章格式IV-*.資格證明文件格式的要求填寫(xiě)和提交有關(guān)資料。
*（*）投標(biāo)產(chǎn)品原產(chǎn)地在關(guān)境外的投標(biāo)人必須是制造商；或者是制造商直接授權(quán)的代理商（進(jìn)口產(chǎn)品開(kāi)標(biāo)前未進(jìn)入中國(guó)關(guān)境內(nèi)的，投標(biāo)人必須是在關(guān)境外注冊(cè)的公司；進(jìn)口產(chǎn)品開(kāi)標(biāo)前已進(jìn)入中國(guó)關(guān)境內(nèi)的，投標(biāo)人必須是在關(guān)境內(nèi)注冊(cè)的公司），代理商投標(biāo)需具備以下條件：
①必須提供制造商對(duì)本次投標(biāo)的唯一正式授權(quán)書(shū)（授權(quán)書(shū)必須由制造商直接授權(quán)，不允許二級(jí)授權(quán)）；授權(quán)書(shū)中應(yīng)包括“制造商對(duì)產(chǎn)品及售后服務(wù)質(zhì)量負(fù)責(zé)”的相關(guān)內(nèi)容和制造商聯(lián)系人及聯(lián)系方式，并確保制造商聯(lián)系人能夠協(xié)調(diào)處理項(xiàng)目執(zhí)行過(guò)程中的重要事項(xiàng)。（對(duì)于此要求，制造商也可在授權(quán)書(shū)外另行出具書(shū)面承諾/說(shuō)明），否則不接受代理投標(biāo)；
②投標(biāo)人在當(dāng)?shù)匾婪ǖ怯涀?cè)的時(shí)間距開(kāi)標(biāo)日滿(mǎn)三年，需提供營(yíng)業(yè)執(zhí)照等注冊(cè)登記的有效證件復(fù)印件；
③距開(kāi)標(biāo)日三年內(nèi)，投標(biāo)人須具有同類(lèi)產(chǎn)品至少兩個(gè)不同年份的銷(xiāo)售業(yè)績(jī)，需提供證明其銷(xiāo)售業(yè)績(jī)的合同或訂單復(fù)印件（至少提供*份不同年度的合同或訂單復(fù)印件）或用戶(hù)證明，并由投標(biāo)人代表簽字或蓋章。
*（*）投標(biāo)產(chǎn)品原產(chǎn)地在關(guān)境內(nèi)的投標(biāo)人必須是制造商（不接受代理商投標(biāo)）。
*（*）投標(biāo)人未被中國(guó)石油招標(biāo)投標(biāo)網(wǎng)(www.cnpcbidding.com)暫停或取消投標(biāo)資格。（根據(jù)《中國(guó)石油天然氣集團(tuán)有限公司投標(biāo)人失信行為管理辦法》的規(guī)定，失信積分達(dá)到*分時(shí)，暫停失信行為人投標(biāo)資格半年；失信積分達(dá)到*分時(shí)，暫停失信行為人投標(biāo)資格一年；失信積分達(dá)到**分時(shí)，暫停失信行為人投標(biāo)資格兩年。暫停期滿(mǎn)后恢復(fù)投標(biāo)資格，有效期內(nèi)的失信記分繼續(xù)執(zhí)行。失信積分超過(guò)**分或在暫停期間再次發(fā)生失信行為的，取消投標(biāo)資格三年。）上述內(nèi)容以評(píng)標(biāo)委員會(huì)現(xiàn)場(chǎng)在對(duì)應(yīng)網(wǎng)站的查詢(xún)結(jié)果為準(zhǔn)。
*（*）投標(biāo)人未被納入中石油集團(tuán)公司供應(yīng)商、承包商、服務(wù)商黑名單（集團(tuán)公司供應(yīng)商、承包商、服務(wù)商黑名單可在中國(guó)石油招標(biāo)投標(biāo)網(wǎng)/www.cnpcbidding.com查詢(xún)）。上述內(nèi)容以評(píng)標(biāo)委員會(huì)現(xiàn)場(chǎng)在對(duì)應(yīng)網(wǎng)站的查詢(xún)結(jié)果為準(zhǔn)。
*（*）距開(kāi)標(biāo)之日前一年內(nèi)，投標(biāo)人在中國(guó)石油物資有限公司組織的招標(biāo)活動(dòng)中對(duì)評(píng)標(biāo)結(jié)果的無(wú)效投訴（包括中石油集團(tuán)公司機(jī)電辦處理結(jié)果為：不予受理、駁回、維持原評(píng)標(biāo)結(jié)果）達(dá)到一次及以上的，投標(biāo)將被否決。
*（*）投標(biāo)人須提供開(kāi)戶(hù)銀行在開(kāi)標(biāo)之日前三個(gè)月內(nèi)開(kāi)具的資信證明原件或該原件的復(fù)印件（由投標(biāo)人代表簽字或蓋章）。
*（*）第八章中要求的其他合格的資格證書(shū)。
(*) All Bidders are requested to fill in according to the requirements of Format IV-* in Section IV and provide relevant information；
(*) The bidder whose product origin is outside the customs must be the manufacturer; Or the agent directly authorized by the manufacturer. The agent shall meet the following conditions for bidding:
① The bidder should provide a letter with original sole official authorized by Manufacturer for the bid；
② the Agent shall get registered in accordance with the local law for at least three years from the bid opening date and copies of valid registration documents such as business license shall be provided;
③ The bidder shall have sales performance for similar products in two different years within three years from the bid opening date. Copies of the contract / purchase order evidencing its sales performance or Certifications from Enduser should be provided and signed or sealed by the Bidder;
(*)The bidder whose product origin is within the territory of the Customs must be the manufacturer (agent bidding is not accepted)
(*) The bidder has not been suspended or cancelled the bidding qualification by petrochina Tendering and Bidding website (www.cnpcbidding.com). (According to the provisions of the Measures for the Administration of Dishonest Behavior of Bidders of China National Petroleum Corporation Limited, the bidder's bidding qualification shall be suspended for half a year if the dishonest score reaches * points; The bidder's bidding qualification shall be suspended for one year if the breach of trust score reaches * points; The bidder's qualification for bidding is suspended for two years after the breach of trust score reaches ** points. If the breach of trust score is **.* points or above, or the breach of trust occurs again during the suspension period, the bidding qualification within the scope of the group company will be cancelled for three years. (*)The Bidder shall provide the original or copy of the credit certificate issued by the bidder's Bank of deposit within three months before the bid opening date (The signature or sealing by the respective of bidders is needed)；
